We have regular briefings too, something I picked up from the military after action debriefs: everyone sits down and the senior most starts out with what "I did wrong and how could I have done it differently".

this is not just when a major mishap has taken place, but at least monthly. "Yesterday I was dropping some old Poplars and went too far in with my back cut, cut my hinge and the durn thing popped back on me as it went down. Dropped where I wanted it , noone was hurt, but I should have been taking my time."

"started up the big saw on teusday, had it on half-choke, I forgot to check the break and got too close to my leg with a running chain on startup."
